For example, Heinze is the ManUtd's player of the year last season.
those prizes are:
Player of the year
Player's player of the year (vote by teammates)
Reserve player of the year
Goal of the season (Goal scored by the Reds of course)

C.Ronaldo got this award with his second goal against Portsmouth away:D
